Compare Submitted Diagnosis Codes with Chart Evidence

Compare submitted diagnosis codes with explicit chart codes for the same member and encounter, creating cited documentation gaps for coding review.

Risk-adjustment review depends on the connection between a submitted code and the underlying clinical record. A code in a billing form is not a substitute for the documentation needed to support it.

Compare submitted diagnosis codes with codes explicitly recorded in clinical documentation for the same member and encounter. Each missing match becomes a coding-review item with the relevant sources attached. This is a code-presence check: it locates potential documentation gaps without inferring diagnoses or deciding whether the record supports a coding or risk-adjustment submission.

1. Set up your account and documents

Sign up for Reducto Studio and get an API key. Set REDUCTO_API_KEY locally, keep it out of source control, and install reductoai==0.24.0 with Python 3.10 or later.

Scope: One member and encounter; submitted codes compared with a clinician-record evidence ledger.

Prepare the 2 input records named submission, chart. Use identifiers from the documents or reviewed supporting records; never invent missing join keys.

2. Extract source values with citations

Parse the document, then pass its job reference into Extract with field citations enabled and chunking disabled. The excerpt below uses the schema and decoding helpers in the full runnable example at the end.

The Parse job reference reuses the parsed document. Citations connect extracted values to source locations; missing values stay null.

3. Compare the extracted fields

Match on the encounter as well as the member. A condition mentioned elsewhere in a longitudinal chart is not automatically evidence for the submitted record being reviewed. Keep the original chart text and source citation alongside the code.

The demonstration compares explicit code presence only. A coder still needs to assess documentation quality, encounter eligibility, provider requirements, and applicable coding guidance. Never add or delete submitted codes merely because this comparison finds a match or gap.

Missing required fields, citation problems, detected role ambiguity, or failed checks return review. checks_clear only means these checks passed, not that the underlying case is approved.

4. Run the example and review the findings

Save the full example as risk-adjustment-documentation-support.py and run python risk-adjustment-documentation-support.py --demo --output demo-results. The synthetic demo checks a consistent case, a discrepancy, and missing data without an API key.

For your own documents, use the manifest, run command in the expandable section below. Each run writes review.json with extracted values, findings, citations, and job references. Use a new output directory and check flagged fields against their cited pages.

Edge cases

  • A claim form and a clinical note are different evidence classes. Do not treat the claim as proof of its own accuracy.
  • Historical, negated, and ruled-out diagnoses need clinical context beyond a string match.
  • Use current coding and model-year guidance before mapping codes into risk categories.

Next steps

Expand the evidence ledger to include provider, signature, document type, and clinical context. Hand the cited discrepancies to qualified coding reviewers before any submission change.

Use a new output directory for each run. review.json contains the source values, checks, citation map, file hashes, and job references. Findings identify a field or parent record through evidence_paths; use the matching citations and source pages to resolve the issue. The script also saves raw Parse, Extract, and any requested Classify responses.

Configure the workflow for your documents

The string paths above use the baseline settings. A manifest value can also hold a file path plus supported options for that document. Optional --classify checks declared roles before extraction; the Classify criteria should distinguish the actual document types in the packet.

Locate form signatures and review dense clinical records

For a scanned form, retain page images and signature-location blocks, then apply Deep Extract to the role with dense clinical text or repeated service lines. The explicit legacy model makes the signatures option meaningful; r-1 handles signature locations natively. A detected signature location is evidence for review, not identity verification or a finding that consent is valid. Image context is useful for interpreting marks that are unclear in text.

Replace the chart entry in manifest.json with this structured entry when the condition above applies. Replace any example page range, sheet name, or prompt with the reviewed selection for your document.
